What is the Purpose of the Survey?
Researchers at the Johns Hopkins University are looking to understand how psychedelic experiences may impact cognitive functioning. We have developed an anonymous survey that takes 20-40 minutes to complete and can be completed on a computer or mobile device. By participating, your responses can help us better understand how psychedelics may impact mood and cognition.

You Are Invited to Anonymously Participate If You...

• Are between the ages of 18 and 74

• Are able to read and write English fluently

• Have had a psychedelic experience that you can recall and describe
